Summer Camp Forms
Hello DREAM Parents! This page provides links to the forms you will
need to fill out in order to send your child to our Summer Camp. Please
check back here in May for links to updated forms and information! For
Summer Camp 2010 dates, please go here.
Don’t forget: You keep all of the forms EXCEPT for the Medical Form, which must be returned to the office in order for your child to come to camp. It is a 3-page form, and the most important parts are the consent to treat and waiver on the second page. Please do not forget to sign these sections, as we cannot treat your child without your signature. Thanks!
